A couple of updates to jQuery CookieBar

We've had some feedback from a few people who are using jQuery CookieBar, and have updated the plugin based on their feedback.

Firstly, we've put a couple of bugs right, so using fixed positioning should work.

Secondly, instead of just being able to append or prepend the CookieBar to the body tag, you can now specify an element by tag, class or id.

And finally, you can specify the effect used once accept or decline has been pressed. The options are slide (The default), fade and hide.

The documentation has been updated accordingly.
